Effective September 30, Colorado Health Medical Group (a/k/a Poudre Valley Medical Group, LLC), has closed our obstetrics and gynecology practice in Fort Collins, formerly Kieft, Cloyd, Hoffmann and Hayes.
Drs. David Cloyd, Mark Hoffmann and Kimberly Hayes are pursuing other opportunities. Dr. Kieft retired in January.
Please know that the choice of healthcare provider is yours. To help you with your decision, you can find a listing of family physicians and ob/gyns is available at pvhs.org.
