Please contact a member of public relations for patient information requests. UC Health follows American Hospital Association guidelines for patient information. When requesting patient information from UC Health, please note the following:

  • Patients have the right to request that no information about them be released.
  • UC Health will not release patient information without the patient's first and last name.
  • UC Health can only provide a brief description of a patient's condition:
    • Undetermined: Patient awaiting physician and assessment.
    • Good: Vital signs are stable and within normal limits. Patient is conscious and comfortable. Indicators are excellent.
    • Fair: Vital signs are stable and within normal limits. Patient is conscious but may be uncomfortable. Indicators are favorable.
    • Serious: Vital signs may be unstable and not within normal limits. Patient is acutely ill. Indicators are questionable.
    • Critical: Vital signs are unstable and not within normal limits. Patient may be unconscious. Indicators are unfavorable.
    • Treated and released: Patient received treatment but was not admitted.
    • Treated and transferred: Received treatment. Transferred to a different facility. PVHS will not disclose where a patient was transferred or the date of the transfer.
